of course it will need to be verified to make sure its all factory, but if it is, this is the 350 powered HD that some felt didnt exist.

if it is a 91, it should be 4 speed, but the shifter looks to be a 5 speed one, so one would need to ask. if it is  5speed, its a swap, or the year is wrong, as the NV4500 wasnt till 1992.

The 8th digit K in the VIN does indicate TBI 5.7 as the engine, and 10th digit M is 1991 so that's pretty good proof. ;D

so that means it is most certainly a sm465 then.

Most likely going to be the NV4500.  I've noticed some people still describe it as a 4 speed since 1st is a granny gear.

The SAE design document on the 3500HD does indicate "The 1991 model year will also see the use of a new 5-speed manual transmission engineered by New Venture Gear."
